model of care assisting individuals to timprove their lives and maximize
their potential to attain their personal goals.

Bell Therapy's
Community Support Programs (CSP's) serve individuals who have a serious
and persistent mental illness and who are living independently or semi-independently
in the community.
How we
serve..
A variety of
services and support are delivered through an intensive, community-based
case management approach designed to maximize growth, independence and
reduce the need for hospitalization.
A client-centered approach
to treatment planning helps ensure that services effectively meet each
client's needs. The treatment team works with each client to develop
a plan that will assist the client in achieving his or her positive personal
outcomes. A strength-based philosophy to treatment helps the treatment
team and the client identify individual strengths that will best assist
the client in achieving his or her life goals, including recovery.
CSP staff coordinate
with each client's support systems including family and significant others,
employers, day programming staff and landlords to ensure continuity of
care.

Our goal...
Our goal is to
provide intensive outpatient services in order to assist clients to remain
living independently within the least restrictive environment.
Referrals
and funding...
Referrals can
come from Service Access to Independent Living (SAIL), a branch of the
MIlwaukee County Behavioral Health Division (414) 257-8095. Referrals
are also made possible through Family Care.
Funding can be
provided through Medicaid Billing, Milwaukee County Contract and Family
Care authorizations.
Admissions requirements
include the following:
- Adults who are seriously and persistently mentally
ill;
- Illness interferes with their ability to manage their
life within the community without treatment intervention and services;
- Follow the criteria for admission that is detailed
in DHS 63;
- Community Support Program admission shall be limited
to an individual who has chronic mental illness which by history
or prognosis requires repeated acute treatment or prolonged periods
of institutional care and who exhibits persistent disability or
impairment in major areas of community living.
