8-bed CBRF serving co-ed adults with chronic mental illness. Limited capacity for non- and semi- ambulatory residents. Home-like setting, family-style meals, activities, transportation, and access to work and day programming are provided. This is a transitional home for individuals either re-entering the community from the hospital or who need additional support to return to a less restrictive living arrangement (e.g. apartment). Treatment focus is on acquisition or reacquisition of essential daily life skills for optimal independent functioning and quality of life. Single rooms are available.
