we are
We
are a community based program that focuses on teaching independent living
skills to individuals thus aiding in successful community living.
Who
we serve
The
Horizons Program primarily serves individuals in the community, who present
with intellectual disabilities, physical limitations and/or mental illnesses
and who are living independently, in a supported apartment, with their
family or relatives and/or in other facilities where services can be provided.
How
we serve
A
variety of services and supports are provided through an assertive, community
based case management approach, designed to increase independence and
quality of life, while reducing the need for hospitalization and/or other
more restrictive services.
A
person-centered approach to treatment and treatment planning is used to
ensure services rendered meet an individual's wants and needs. The
treatment team works with each consumer in the development of a plan that
addresses his or her desired outcome. A strength based philosophy
of treatment helps the consumer and treatment team identify and accomplish
consumer outcomes.
Each
Horizons Program staff coordinates with family members, guardians, other
treatment providers, landlords and employers to assist with continuity
of services.

Our
goal
Our
goal is to provide comprehensive, community based services in order to
assist consumers to acquire and/or retain skills to continue to live as
independently as possible within the least restrictive environment.
Referrals
and funding sources
Referrals
and funding can come from the Milwaukee County Disabilities Service Division,
the Milwaukee County Department of Family Care (MCDFC), Community Care,
IRIS and iCare as well as other Family Care organizations or private pay.
Admissions
requirements include the following:
- Individuals 18
years or older
- Diagnosis of a
intellectual, developmental, mental or physical disability
- Persons demonstrating
a need for multiple services
- Independent or
Semi-independent living situation
