Our facilities
are coordinated around the following options:
- 14, 8-bed CBRF's
- 3, 6-bed CBRF's
- 1 large, 46-bed CBRF
- 1, 4-bed AFH
Staffing patterns
include 24 hour coverage at all residential facilities. Residential
staff are trained to provide person-centered and recovery oriented care
that focus on comprehensive support, supervision and training for all
individuals. Our facilities are located in neighborhoods throughout
Milwaukee.

Homes (CBRF & AFH)
Serious
& Persistent Mental Illness Homes (MI):
We provide comprehensive,
community-based residential treatment to adult men and women with severe
and persistent mental illness, who may also carry a diagnosis of mild
intellectual disability, substance use and/or abuse, and traumatic stress,
all of which have seriously impaired their quality of life for extended
periods of time. We provide 24 hour staff to support, assist and
teach consumers basic daily living skills.
Intellectually
& Developmentally Disabled (IDD) Homes:
We provide comprehensive,
community-based residential treatment to adult men and women with intellectual
and developmental disabilities. Our focus is to assist these individuals
who have historically experienced significant challenges with adaptive
functioning and behavioral controls. The objective is to provide
a living environment which emphasizes consistency, daily repetition, structured
learning, active participation in daily living skills, alleviation of
maladaptive behaviors and positive role modeling.
Serious and Persistent
Mental Illness Respite Home:
Our respite home serves
individuals with serious and persistent mental illness as an alternative
to psychiatric inpatient hospitalization that offers a continuum of community-based
services and intervention and support to individuals in crisis.
Our aim is to work with consumers in crisis by helping them stabilize
their behaviors so that a return to a less restrictive environment is
possible. The respite home offers normalcy inside of an intensive,
therapeutic environment that helps to support individuals experiencing
pyshological crisis on an acute basis.
Intellectually
and Developmentally Disabled Crisis Respite Home:
Our respite homes for
individuals with Intellectual and Developental Disabilities serve adult
consumers who are mildly to moderately intellectually or cognitively disabled
and are in urgent need of temporary placement due to a disruption in their
lives or an acute crisis. The respite home provides the opportunity
for these consumers to receive prompt services while still remaining in
the community and avoiding hospitalization. Our consumers can remain
engaged in their normal daily activities while they remain at the respite
home. The respite home also offers a safe transition for those individuals
coming from the hospital to the community.

Our goal is to
design services to enhance and teach each individual entrusted to our
care, the skills needed to achieve greater independence.

Funding
opportunities can be provided through Milwaukee County Department of Family
Care, Community Care, Milwaukee County SAIL, Milwaukee COunty Disabilities
Services Division, IRIS, Department of Corrections, Wraparound Milwaukee,
Private Insurance and Private Pay.
