Who
we are...
We
are a multi-faceted, dually-licensed Child Care Institution and a Licensed
Child Placing Agency (LCPA). This status affords us the opportunity
to develop a variety of programs, each of which is governed by state regulatory
standards and implemented in accordance with policies and procedures established
by Phoenix Care Systems, Inc., (PCS) our parent body.
Our
program is best described as "Residential without Walls".
The physical environment presents and transmits a home-like atmosphere.
Although our setting is uniquely configured in the liking of a small college
campus, the environment is highly structured with the presentation of
one to four (1:4) staff to consumer ratio, and with 24 hours awake coverage.
The living units are locked and secured from the outside to ensure that
no outsider will intrude.
Who
we serve...
We
serve male and female children and adolescents, ages 8 to 21 years, with
emotional, behavioral, cognitive and/or learning disorders. These,
our primary consumers, are generally wards of a Child Welfare and/or Juvenile
Justice System. We also serve referrals from the local Department
of Education in the State of Indiana as well as neighboring states.
How
we serve...
We
serve by performing 15 minute face to face bed checks to ensure that each
consumer is accounted for during every shift. In the event that
more intensive services are required, we offer one to one (1:1) staff
to consumer ratio. Staff is responsible for being within arm reach
of the consumer at all times on their shift.
Each
consumer receives an initial assessment at intake and quarterly thereafter.
These assessments identify needs and evaluates goal(s) attainment.
Individual Service Plans (ISP) are developed that include the scope of
therapeutic interventions such as:
- Individual Therapy,
- Family Therapy
(where appropriate),
- Group Therapy,
- Recreation Therapy,
- Psychiatric Consultation,
Evaluation and Medication Management
Our
goal...
Our
goal is to provide each consumer with a premier therapeutic experience.
We are committed to facilitating the involvement of their family (when
possible and/or appropriate), and to working collaboratively with referral
sources toward reunification or preparation for independent living.

Funding
opportunities are made possible through the Indiana State Board of Education
(ISBE), Indiana Department of Child Services and Juvenile Justice System
and other referral sources in surrounding areas.

Home: Transitional Living Skills Program
Who
we are...
We
are a multi-faceted, dually-licensed Child Care Institution and a Licensed
Child Placing Agency (LCPA). This status affords us the opportunity
to develop a variety of programs, each of which is governed by state regulatory
standards and implemented in accordance with policies and procedures established
by Phoenix Care Systems, Inc. (PCS), our parent body.
Our
program is located off of our main campus in a typical residential neighborhood.
It offers services intended to assist consumers in their late teens to
plan and prepare to move forward upon graduation to an independent living
situation. In the event of a consumer requiring a more intense level
of care, a transfer to our residential program is made for stabilization.
Who
we serve...
We
serve female adolescents, ages 16 and older, with emotional, behavioral,
cognitive and/or learning disorders, however, these consumers will have
made significant improvement in their daily functioning prior to making
the transition.
How
to serve...
We
serve our consumers through instruction in the following domains:
- Self Development,
- Communication/Social
Skills,
- Daily Living Skills,
- Housing Management,
- Money Management/Budgeting,
- Career Planning,
- Academic Support,
- Community Resources,
and
- Transportation,
Each
