Community Care provides care coordination for court-ordered children
and their families through Wraparound Milwaukee. Willowglen Academy-Wisconsin
Community Care serves as the facilitation link for providing community-based,
mental health services for children and their families.
Care coordinators build and develop child and family teams to
identify strengths, assess needs and develop strategies to help
individuals achieve an overall family vision. This is accomplished
in a child-centered, family-focused manner that ensures an individualized
care plan and a voice in all decisions.
By providing comprehensive care services and oversight, care
coordinators are able to keep their children and families united
within the home and the community.
